If you are off work sick for seven days or less, your employer should not ask you for a doctor's certificate. You will need a medical certificate if you are off work sick for than seven days. The seven days include days that you don't normally work. So when you work out how long you've been off sick, you should include weekends and bank holidays.
You can request your Fit Note using the forms below or via our online service form or by calling the surgery on 01283 703318. If you have not seen a clinician at the surgery for this illness / condition then on most occassions you will be required to do so.
If you are requesting a Fit Note after a hospital procedure you will need to go back to your hospital team. They should supply a Fit Note for the full time you require off.
